If I have had (a) seizure or seizures, what do I need to know?

If you have a history of seizures. Please visit the FMCSA Website for more valuable information on this topic. Visit this link to view the FMCSA Seizure Exemption Application.

NOTE: You will NOT send this application into the FMCSA for exemption consideration UNTIL you have the MEC form signed and confirmed by the CME at Bellows Health Systems that otherwise you meet or exceed the requirements of a healthy and safe CMV driver.

What age do I have to be to take the DOT with Medical Card physical?

18 Years of age is the minimum age to take the FMCSA MEC (DOT) examination.

If my vision is < than 20/40 in my worse eye, what do I need to know?

There is an Alternative Vision Standard that is available for the driver who has monocular vision < 20/40 vision in the worse eye). This will require a collaborative effort including the Certified Medical Examiner at Bellows Health Systems and either an optometrist or ophthalmologist. The form that is needed will be: Vision Evaluation Report Form: FMCSA Form MCSA-5871 (dot.gov)

Form MCSA-5871 cannot be submitted to the CME more than 45 days after the optometrist/ophthalmologist signs it.

What do I need to know if I take insulin for diabetes?

There is a collaborative effort between the Certified Medical Examiner at Bellows Health Systems and your PCP or specialist that prescribes and monitors your Insulin and any other diabetic medications. The form Insulin-Treated Diabetes Mellitus Assessment Form MCSA-5870 (dot.gov) will have to be completed and signed by your PCP or specialist not more than 45 days prior to your appointment with the CME at Bellows Health Systems.

If I have deafness in one or both ears, what do I need to know?

If hearing from one ear, the CME at Bellows Health Systems may be able to qualify you to drive a commercial vehicle, provided that you possess adequate hearing in the one ear. Candidates that are deaf in both ears have an opportunity to possess an exemption for hearing from the FMCSA Form MCSA-5876 (dot.gov) with the box checked, “Accompanied by a waiver/exemption” with the specified type, written or printed on the corresponding line.

If I have a fixed deficit (missing or impaired limbs), do I need a Skill Performance Evaluation (SPE)?

Once the driver obtains the MEC (MCSA-5876) card from the CME at Bellows Health Systems with the box checked “Accompanied by a Skill Performance Evaluation (SPE) Certificate”, he or she can apply for an SPE Examination. Visit the FMCSA’s website at: Skill Performance Evaluation Certificate Program | FMCSA (dot.gov), to find an SPE service center.

What general items, information or preparedness should I bring to the exam?

  • You are required to bring a form of photo identification.

  • You will need the ability to deliver a urine sample for a kidney screen (This is NOT a Drug Test).

  • Please bring with you a list of your medications, dosages, and Name of your PCP/Specialist, Healthcare System and location.

  • Please wear comfortable, loose-fitting clothes. Drivers are NEVER gowned. However, the Bellows Health Systems’ doctor is going to examine you, so dress accordingly.

  • It is not recommended to consume items like energy drinks, coffee, sod, and tobacco products prior to the examination, especially if you have borderline hypertension or white coat syndrome.

If my address differs from the listing on my identification, which address should I put on the form?

Please put the location of your current domicile when you complete the MCSA-5875 form.

If I have been prescribed a CPAP or BIPAP device for use, what do I need to do?

Bring to the examination, the most recent 60 Day Usage Report for any CPAP or BIPAP if prescribed by a PCP or Specialist.

If I have been prescribed or use Schedule I, II, or lll medications, what do I need to know?

Download, print, and complete FMCSA Form MCSA-5895 (dot.gov) for any medications that are schedule I, II, or lll. This form cannot be signed by the PCP or Specialist more than 45 days prior to the examination with Bellows Health Systems CME. The examiner may request additional MCSA-5895 forms to be completed depending on the presenting situation, medication and dosage. That additional request would be determined at time of visit upon review of your case.
